New patients can print and complete the patient forms prior to your appointment with Dr. Baker to save time. If you cannot print the form from the website, please call our office to have one emailed to your address. If you have not completed the forms prior to your first appointment with Dr. Baker, please arrive 20 minutes early to complete the forms in the waiting room prior to seeing the doctor.
